Meet Our Team

Dr. Forman received his Doctor of Dental Medicine (D.M.D.) from Harvard University, where he graduated Cum Laude with Honors in Research. In his final year he was inducted into the prestigious Omicron Kappa Upsilon (OKU) honor society, representing the top five students of his graduating class. An active researcher at Harvard, he received multiple research grants resulting in numerous peer-reviewed publications and presentations at national and international conferences, along with awards including the AAOMS Implant Award and the Dr. Norman B. Nesbett Award for overall excellence in clinical dentistry.

After dental school, Dr. Forman returned to the tri-state area to complete a rigorous six-year Oral and Maxillofacial Surgery residency at Columbia University Medical Center–New York Presbyterian Hospital. During that time he earned his Medical Degree (M.D.) from Columbia University, College of Physicians and Surgeons, completed a full year of general surgery training, and arranged rotations at some of the country’s busiest trauma centers. In his final year he served as Chief Resident of the Department of Oral and Maxillofacial Surgery.

Dr. Forman enjoys all aspects of Oral and Maxillofacial Surgery, with a strong passion for dental implant surgery, wisdom tooth removal, dentoalveolar surgery, orthognathic (corrective jaw) surgery, guided bone regeneration, maxillofacial reconstruction and trauma, pathology, management of TMJ dysfunction, full mouth rehabilitation, and sleep apnea. He has a personal interest in using cutting-edge technology — 3D printing, guided surgery, and virtual surgical planning — to deliver precise care, and has over 20 peer-reviewed publications and textbook chapters in the field.

He became a doctor for one simple reason — to follow his passion to help others. He strives to deliver patient-centered, evidence-based treatment, understanding that each patient is unique. Outside of work, Dr. Forman enjoys being part of the community, golfing, and spending time with his wife, Rebecca, and two children.

Dr. Brett Zuckman returned to his home state of Connecticut to practice at Greenwich Oral and Maxillofacial Surgery, bringing deep community roots as a Fairfield County native along with comprehensive surgical training from his four-year residency at Lincoln Medical and Mental Health Center in New York City, where he served as Chief Resident in his final year.

Board certified as a Diplomate of the American Board of Oral and Maxillofacial Surgery and a Fellow of the American Association of Oral and Maxillofacial Surgery, Dr. Zuckman specializes in wisdom tooth removal, pediatric oral surgery, extensive oral and facial prosthetic rehabilitation using bone grafting and dental implants, and maxillofacial trauma involving both hard and soft tissue.

He serves as an attending physician at Greenwich Hospital, where he takes trauma calls for the Emergency Department, an associate clinical professor at Columbia University, and an attending surgeon at New York Presbyterian Hospital. Dr. Zuckman currently resides in Westport, CT with his wife, Martha, and their three children.

Dr. David Salomon serves as Clinical Instructor of Surgery at Yale University School of Medicine and is on staff at Yale-New Haven Hospital. He directs resident research, serves on admissions and clinical competency committees, and actively mentors residents — contributions that earned him the Bernard Levine Attending of the Year Award in both 2023 and 2024.

Dr. Salomon brought advanced treatments to southern Connecticut, including trigeminal nerve repair, facial bone reconstruction using tissue engineering, and zygomatic implants. He is a Fellow of the American College of Surgeons and has been recognized as “Best Dentist in Connecticut” by Connecticut Magazine for seven consecutive years.
